• Open application attachment below, click on "File" at top left of Toolbar and then " Save As" a pdf file to your computer
• From your computer then open the EZ App pdf file you have saved with Adobe Reader 9 ( Free Download Available)
• Begin entering data right from your computer keyboard-the Adobe Reader 9 format allows the data to be saved meaning you have permanent data file application you can reuse when needed Once completed please sign and date page 6, close the attachment and email it directly back to Trinity Physician Insurance for fast processing.
